2012 Open Enrollment

Welcome to Open Enrollment for selection of your 2012 insurance benefits. Open Enrollment is your opportunity to review your benefits and make any changes necessary to ensure your choices continue to meet your needs.

WHEN:  November 1st - November 20th, 2011
HOW:  Online via ESS (Employee Self Service)

After November 20th, contact Human Resources to make changes or corrections.

New to Dallas County:

Optional Life Insurance Enhancements

Beginning in 2012, the optional life insurance cap will be increased from $200,000 to $400,000.  (for example, if the employee has 2 times salary optional life and was previously capped at $200,000, the new coverage amount will automatically increase up to the maximum $400,000 amount).

Beginning in 2012, spouse optional life insurance is available at five different levels. $10,000 or $25,000 without Evidence of Insurability; and $50,000, $75,000 or $100,000 with evidence of insurability

Medical Plan Changes

Emergency Room Co-pay (EPO/PPO) increased to $150 (was $100).

In-Network PPO deductible will increase –
$500 individual (was $250)
$1,000 family (was $500)

Out-of-Pocket maximum EPO
$2,000 individual (was $1,750)
$4,000 family (was $3,500)

Out-of-Pocket maximum PPO
$3,000 individual (was $2,750)
$6,000 family (was $5,500)

Smoker’s Rates/Non-Smoker Rates

If you or a covered family members smoke, you will pay a higher medical premium for 2012 medical plan coverage. To claim the non-smoker rates in 2012- you must log into Employee Self Service (ESS) and disclose if you are a smoker or a non-smoker.

You can also claim the non-smokers rates if you and your covered family members are smoke free by January 1, 2012. If you complete an approved smokers program by June 1, 2012 you can claim the non- smokers rates for the remainder of 2012.

Audit of Dependents
The Human Resources/Civil Service will be conducting audits of covered dependents.  Make sure your dependent is eligible for coverage.  Employees who are covering ineligible dependents may be subject to disciplinary action.
