Execute
this form in duplicate. Mail the original, the duplicate,
a self addressed stamped envelope and the required fee of
$14.00 for the first signature (owner) and .50 for each signature
(owner) thereafter. If all owners appear in person with their
valid government picture ID the County Clerk deputy can take
a deputy acknowledgment for an additional $2.00 fee in place
of a notary acknowledgment. Otherwise, all signatures will
need to be notarized.
