The following may apply for a Birth Certificate:
Individuals requesting or picking up a birth certificate must be qualified applicants. Qualified applicants consist of (self, parent, grandparent, or sibling). Anyone other than the qualified applicants must have a signed (original) statement written by one of the above qualified applicants authorizing them (naming them) to pick up the birth certificate. This statement must include a copy of the qualified applicant's government issued ID. The person authorized to pick up the birth certificate will also need to present their valid government issued ID.
Please mark on your request application if you are wanting the Long Form Birth Certificate or the Short (Abstract) Form Birth Certificate.
Long Form Birth Certificate:
The long form birth certificate is the original birth certificate which contains detailed information.
- person's name, sex, date of birth, and place of birth.
- parent(s) name, date of birth and place of birth.
- name of the hospital, doctor and time of birth.
- State File Number and the Local File Number that the record has been registered with.
The long form birth certificate is usually needed for newborns, passport applications and Indian Registry. Our office only provides long form birth certificates for births that occurred in Dallas County excluding the city limits of Dallas ( eg. Garland, Mesquite, Richardson, Balch Springs, DeSoto, Duncanville, Lancaster, Irving, Las Colinas, etc).

Short (Abstract) Form Birth Certificate:
The short (abstract) form birth certificate contains basic information.
- person's name, sex, date of birth, and county of birth.
- parent's name(s), the State File Number and the filing date.
The short (abstract) form can be used to apply for a government issued ID, enrollment in school or sports, and for work related issues. When applying, please indicate the type of form you 're requesting as no refunds will be issued.
