A National Vision -The Hidden Epidemic - Confronting Sexually Transmitted Diseases, was published in 1997 by the Institute of Medicine, National Academy of  Science, and states, AOf the top ten most frequently reported diseases in 1995 in the United States, five are STDs. Rates of curable STDs in the United States are the highest in the developed world and are higher than in some developing regions.

Conclusions and Recommendations

  • An effective national system for STD prevention to be established in the United States.
  • Establish a national system for STD prevention, the committee recommends four major strategies for public and private sector policy makers at the local, state, and national levels:
    • Overcome barriers to adoption of healthy sexual behaviors.
    • Develop strong leadership, strengthen investment, and improve information systems for STD prevention.
    • Design and implement essential STD-related services in innovative ways for adolescents and underserved populations.
    • Ensure access to and quality of essential clinical services for STDs.

A Regional Vision - The South Central STD/HIV Prevention Training Center in Dallas, Texas, and Little Rock, Arkansas, encourages the utilization of physicians and physician extender clinicians (i.e., PAs, NPs and nurses) functioning under written protocol, and in accord with current CDC, STD Consensus Guidelines, and under the supervision of the physician for the prompt evaluation of abnormal findings, diagnosis, treatments, etc. Delay of therapy of STD infection significantly increases the likelihood of development of sequelae, and the fiscal and human costs of these common infections.

In addition, we promote the concept of a STD clinic with an integral "stat" laboratory to ensure rapid, objective detection of the presence of STDs whenever possible. STDs commonly are asymptomatic in their clinical presentation, and often present with "atypical signs and symptoms" of disease. The strong social stigmata associated with STDs of all types may result in patient and institutionalized behaviors that result in delay of treatment, or allowance of the patient to develop sequelae before receiving adequate medical and epidemiological management.
