Mother Father Spouse Sibling Child - 18 and over Maternal Grandparents Paternal Grandparents Anyone other than the qualified applicants MUST have a signed (original) statement written by one of the above qualified applicant’s authorizing them by name to pick up the birth certificate. This statement MUST include a copy of the qualified applicant’s VALID government issued ID. The person authorized to pick up the birth certificate will also need to present their VALID government issued ID. IDENTIFICATION REQUIRED:
FORMS OF IDENTIFICATION CANNOT BE MUTILATED, TORN, TAPED TOGETHER OR LIMITED The cost is $23.00 in CASH or MONEY ORDER ONLY Long Form Birth Certificate If the long birth certificate is needed (required for passports) the Dallas County Clerk’s office can issue certificates for Dallas County births that occurred outside the City limits of Dallas (eg. Grand Prairie, DeSoto, Duncanville, Lancaster, Irving, Las Colinas, etc). Go to: for information on securing a long form birth certificate.
