Abstract copy of birth certificate

Our office provides abstract copies of birth certificates for the state of Texas, if you were born in a hospital. 

In order to request a birth certificate you must be one of the following:

• Self
• Parent
• Step Parent
• Grandparent
• Sibling

If not one of the above, the requester must have:

• A signed original statement authorizing and naming them to pick up birth certificates.

•  Statement must include a copy of your government issued identification with the person named in your request.

 Prison/Jail identification will not be accepted.

Long birth certificate

If the long form birth certificate is needed, our office only provides certificates for births that occurred outside the City limits of Dallas (eg. Garland, Mesquite, Richardson, Balch Springs, DeSoto, Duncanville, Lancaster, Irving, Las Colinas, etc).

If you were born within the Dallas County city limits please contact the City Bureau of Vital Statistics @ (214) 670-3248.

All requests done by mail will automatically receive the abstract version.

Fees

Cost per copy is $23.00, make money order payable to the County Clerk of Dallas.  We do not accept personal checks.  Please do not send cash through the mail.  Turn around time is 10 to 15 business days.  For more information call 214-653-7478 or 214-653-7007.

Application For A Certified Copy Of Birth Certificate

The following information is required:

• Full name as appears on birth record
• Date and Place of birth: If the birth occurred within the last 90 days, within the City Limits of Dallas, you will need to contact the City Bureau of Vital Statistics at (214) 670-3248.
• Parents name including mothers maiden name
• Your relationship to person named on certificate
• Your mailing address
• Purpose of record In writing you will need to send a copy of your valid government issued I. D.
• In person you will need to provide your original valid government issued I. D.

The above information is required by statute. Birth records are considered confidential information; therefore all requirements must be met. If you are unable to meet the requirements, please contact the State Bureau of Vital Statistics in Austin.
