On this web page, you will find information on: (1) what you need to do prior to enrolling and (2) any 2022 plan changes. Colonial Life Insurance

It’s more important than ever to prepare for the unexpected. During Open Enrollment most voluntary benefits will have guaranteed Benefit options.

Benefit Counselors will be available to review your options via virtual 1-on-1 meetings, telephonic session with our call center, or you can self-enroll on their website. They are available to answer questions, assist with claims, and provide additional information.

Dallas County employees are eligible for a complimentary $10,000 Accidental Death & Dismemberment. Speak to a Colonial Benefit Counselor to receive your certificate.

The Hartford Life Insurance
Increasing life insurance can only be submitted during Open Enrollment. The deadline to submit the application to HR Benefits is November 30, 2021. Once approved, you will be notified through The Hartford.

HR Benefits does NOT store or have access to your Beneficiaries.

Open Enrollment gives you the annual opportunity to make changes to your benefit elections.

WHEN:  October 18th - November 12th, 2021
HOW:  Online via ESS (Employee Self Service)

Beginning on October 18, 2021, you will be able to make changes on-line by utilizing ESS. Changes to medical, dental, vision, flexible spending accounts, dependent life insurance, etc.  After November 12th, no corrections can be made. Print a copy of your confirmation for your records; this is a requirement AND your sole proof of your confirmed elections if a systems error occurs resulting in incorrect benefits enrollment.

Any changes you make during Open Enrollment will take effect January 1, 2022.

Surcharge Affidavit: To be completed if you are enrolling your spouse in your employer medical plan (PPO Plan or HDP, including retiree PMD/PSD). If you are NOT enrolling your spouse in your employer medical plan you do not need to complete the electronic submission. If you fail to complete the electronic submission or are late submitting it and you enroll your spouse in your employer medical plan, a $200.00 per month surcharge will be assessed every month until the electronic submission is completed. NO REFUNDS!
