The Human Resources/Civil Service Department is pleased to provide this resource to assist you, our employees, with your planning and preparation for Open Enrollment for Plan Year 2020. On this web page, you will find information on: (1) what you need to do prior to enrolling and (2) any 2020 plan changes. We encourage you to spend time closely reviewing the general information and plan changes before you enroll. By preparing for your enrollment, your experience will be fast and easy!

Open Enrollment gives you the annual opportunity to make changes to your benefit elections.

WHEN:  October 28th - November 8th, 2019
HOW:  Online via ESS (Employee Self Service)

Beginning on October 28, 2019, you will be able to make changes on-line by utilizing ESS. Changes to medical, dental, vision, flexible spending accounts, life insurance, etc.  After November 8th, no corrections can be made. Print a copy of your confirmation for your records.

Any changes you make during Open Enrollment will take effect January 1, 2020.

Do you have questions about the different plans?

Cigna, United Health Care (UHC) and CVS Caremark will be onsite at the informational meetings with a special presentation for Dallas County employees.

Surcharge Affidavit: To be completed if you are enrolling your spouse in your employer medical plan (PPO Plan or HDP, including retiree PMD/PSD). If you are NOT enrolling your spouse in your employer medical plan you do not need to complete the electronic submission. If you fail to complete the electronic submission or are late submitting it and you enroll your spouse in your employer medical plan, a $200.00 per month surcharge will be assessed every month until the electronic submission is completed. NO REFUNDS!

2. Select "Spouse Surcharge Affidavit form" from the left hand menu.
3. Enter your Log-in ID and password.
4. Enter your SSN (with dashes) and your spouse's birthday (please note, your spouse must be in your contacts in order to populate).

5. If you've previously submitted a Spouse Surcharge Affidavit, it will pull up for you to verify the information.

6. If you've never entered a spouse before, you'll get a red message:

7. Click on the "Submit New Affidavit" button and follow the prompts to complete and submit the affidavit.
