What conditions require mandatory notification?

Notifiable disease conditions required by the Texas Health and Safety Code can be found here:

How do I report a notifiable condition?

To report a notifiable condition (suspected or confirmed) or a public health emergency, contact us:

• During normal business hours:
  • Call (214) 819-2004 to report all conditions except tuberculosis and sexually transmitted diseases.
  • Call (214) 819-2060 to report tuberculosis. Click here for more information.
  • Call (214) 819-2155 to report sexually transmitted diseases. Click here for fax and mail instructions.
• After business hours:
  • Call (214) 235-1799 to report all conditions.
