The Dallas County Healthy Start Program’s Care Coordination Team provides personalized, one-on-one support to help women, infants, and families navigate every stage of pregnancy, postpartum, and early childhood. Our dedicated team of certified community health workers connects participants to essential services, from prenatal care and postpartum support to infant care guidance, mental health resources, and nutritional assistance. We also offer referrals for healthcare, housing, transportation, and other community resources, while ensuring cultural and language needs are met. Through compassionate care and tailored guidance, we work alongside families to promote health, well-being, and a strong start in life. Appointments can be scheduled in the office or in your own home.
