You can download and print the Complaint Form and write an Informal letter to the court.

Your complaint should include:

• Your name and contact information.
• The name of the guardian and the person under guardianship.
• A detailed description of your concerns, including specific actions or inactions of the guardian, dates, and any relevant incidents.

Include any supporting documents that substantiate your claims.

• Examples:
  • Medical Records
  • Police Reports or investigation reports
  • Court Orders or legal paperwork
  • Case records, witnesses with contact information
  • Any other relevant information, material, recordings, or records relevant to the complaint/concerns.

The court offers three ways for you to submit your complaint to the court.

• Call: (214) 653-6533. If you call in your complaint, you will still be required to complete a complaint form with the Investigator assigned.
  • This option is for those without computer access

An investigator will be assigned to address your complaint. The investigator will contact you within 48 hours of your complaint being filed to further discuss your concerns and initiate a preliminary investigation. If you have not heard from an investigator, you may call (214) 653-6533 to check the status of your complaint.

Not every investigation will result in judicial intervention. The Investigator will review your complaint and supporting documentation. The case will be staffed with the counsel for the court investigators’ office. If further investigation is warranted, the assigned investigator will continue the case. Case CANNOT be discussed while the investigations are ongoing. The assigned investigator will submit a Findings and Conclusions report with the court. Any judicial intervention will be at the discretion of the court. The court has several options at their disposal:

• Assign a Guardian Ad Litem
• Order a Show Cause/Status Conference
• Dismiss the complaint if it lacks merit
