New laws effecting the purchase of Birth Records
September 1, 2015
Health and Safety Code 191.0031 | Certified copies by mail | The state registrar may not issue a certified copy of a record under this chapter to a person who has applied for the record by mail unless the person has provided notarized proof of identity in accordance with rules adopted by the executive commissioner of the health and Human Service Commission. The rules require the issuer of the certified copy to verify the notarization using the records of the secretary of state under Section 406.012, Government Code. |
Effective June 1, 2015 the Dallas County Clerk's Offices will no longer
accept the Matricula Consular Card as verification of identity for purchase of birth certificates or for obtaining confidential records.

Short (Abstract) Form Birth Certificate
The short (abstract) form birth certificate contains basic information such as:
- person's name, sex, date of birth, and county of birth.
- parent's name(s), the State File Number and the filing date.
The short (abstract) form can be used to apply for a government issued ID, enrollment in school or sports, and for work related issues. When applying, please indicate the type of form you are requesting as no refunds will be issued.
Long Form Birth Certificate
The long form birth certificate is the copy of the actual certificate of birth on file with the locality where the birth occurred.
It is not available for the City of Dallas or other counties.
Long Form Birth Certificates are only available for the following Dallas County Cities:

- Dallas Veterans Affairs Medical
- Renaissance Hospital Dallas

- Lakepoint Hospital (Rockwall County)
- Medical City Dallas
- Trinity Medical (None Before-1996)
- UT Southwestern

Application For a Certified Copy of Birth Certificate |
The following information is required:
- Full name as it appears on the birth record
- Date and county of birth
- Parent(s) name, including mother's maiden name
- Your relationship to the person named on the certificate
- Your mailing address
- Purpose of record
- Full name of hospital where the birth occurred
- In person, you will need to provide your original valid government issued ID
- In writing, you will need to send a notarized copy of your valid government issued ID
- Cost per copy is $23.03 money order, payable to John F. Warren, Dallas County Clerk’s Office
- A NON-REFUNDABLE SEARCH FEE OF $23.03 WILL APPLY FOR EACH RECORD NOT FOUND
- Turn around time is 2 to 4 weeks when applying by mail please send money order only. If applying in person, you may pay in cash and receive the record while you wait.
- Credit cards will not be accepted as a form of payment by mail.

Birth records are considered confidential information; therefore, all requirements must be met.
