Workers Compensation Injured on Job Employee

I was injured on the job. What should I do?

Emergency

  • Call 9-1-1 immediately.

  • If you are incapacitated, a supervisor or co-worker should call 9-1-1.

  • Your supervisor will call “Company Nurse On Call” at 1-888-770-0931 and report your on-the-job injury no later than the end of the shift or within 24 hours.

  • After you are stabilized, you must call “Company Nurse On Call” to provide your statement of the incident. “Company Nurse On Call” will complete the Incident Report (First Report of Injury, DWC-1).

  • The injured employee and supervisor will fill out the 504 Dallas County’s Workers Compensation acknowledgment form and fax it to the Human Resources Workers Compensation Department.

Non-Emergency

  • For all non-emergency on-the-job injuries (regardless of how minor the injury), you and your supervisor must call “Company Nurse on Call” at 1-888-770-0931 and report your injury immediately.

  • The injured employee and supervisor will fill out the 504 Dallas County’s Workers Compensation acknowledgment form and fax it to the Human Resources Workers Compensation Department.

  • “Company Nurse on Call” will determine the extent of your injury and if first aid or medical attention is required. “Company Nurse On Call” will complete the Incident Report (First Report of Injury, DWC-1).

  • First aid only required – The nurse will instruct you on how to treat the injury and tell you to contact “Company Nurse” if the injury worsens.

  • Medical attention is required – You will be referred to the nearest workers’ compensation facility for treatment (near your home or work location). Note: You should evaluate your condition and ability to go to the medical facility without assistance and request assistance if necessary.

Reminder: Call “Company Nurse on Call” immediately when an injury occurs or as soon as possible, preferably no later than the end of your shift or within 24 hours.
