Assumed Name Applications may be completed online or in office via our research library computers. We do not accept personal checks; money order, cashier’s checks or credit cards only.

How to Apply for a Withdrawal

In order to properly withdraw or remove an individual from a business please follow the steps below:

  1. Mail the original withdrawal form, a self addressed stamped envelope, and the required fee of $24.00 for the first signature (owner) and .50¢ for each signature (owner) thereafter.
  2. If all owners appear in person with their valid government picture ID the County Clerk deputy can take a deputy acknowledgment for an additional $2.00 fee in place of a notary acknowledgment. Otherwise, all signatures will need to be notarized.

Please note:

  1. All owner's signatures must be notarized. We do not have notary's available in our office. However, a County Clerk deputy can take a deputy acknowledgment with all owner's present. Each owner must have a valid government issued picture ID. The cost is an additional $2.00 per form.
  2. The boxed area titled "certificate of ownership, names of owners" requires your name to be printed, your original signature and your residential address (no post office box in this area).
  3. The original document will be returned to you once it is properly recorded. If you are needing copies the copy cost is $1.00 per page and an additional $5.00 for certification. Fees must be included with the request for copies.
